The Health Home program offers whole-person care coordination for individuals navigating both behavioral and physical health challenges. Our team works with each client to connect them to the medical, behavioral, and social supports needed to thrive. It's not a physical home—it's a network of wraparound services designed to keep you healthier and better supported.

Each client is assigned a care manager who creates and oversees a person-centered care plan. We use electronic health records and the eMedicaid system to ensure all providers involved in a client’s care are aligned and informed.
Clients may be referred by providers, community agencies, or self-enroll if eligible. Our team will assess eligibility and help complete the necessary forms to begin care coordination services.

Insurance: We accept Maryland Medicaid. Self-pay options are also available.
