Who This Program Is For
Adults and children who are Medicaid-eligible and have serious mental illness (SMI), serious emotional disturbance (SED), or opioid use disorder—especially those at risk for chronic medical conditions.
Program Overview
The Health Home program offers whole-person care coordination for individuals navigating both behavioral and physical health challenges. Our team works with each client to connect them to the medical, behavioral, and social supports needed to thrive. It's not a physical home—it's a network of wraparound services designed to keep you healthier and better supported.

Comprehensive care coordination
Medical appointment scheduling & follow-up
Health promotion and disease management education
Behavioral health and medication management integration
Connection to housing, food, and transportation support
Crisis prevention planning
Linkage to primary care and specialty providers
How It Works
Each client is assigned a care manager who creates and oversees a person-centered care plan. We use electronic health records and the eMedicaid system to ensure all providers involved in a client’s care are aligned and informed.
Program Information
How to Enroll or Refer
Clients may be referred by providers, community agencies, or self-enroll if eligible. Our team will assess eligibility and help complete the necessary forms to begin care coordination services.
Location, Hours & Insurance
Location: 413 Eastern Blvd, Essex, MD 21221
Hours: Monday–Friday, 9 AM–5 PM
Insurance: We accept Maryland Medicaid. Self-pay options are also available.
